CONNECT establishes an operational process for oncology navigation teams to track, deliver, and document supportive care services to cancer patients. Patient-centric workflows for distress screenings, barriers to care, and care plan delivery keep the navigation team focused on navigation best practice standards, ensuring that the process is consistent for all patients. The platform provides an environment for the navigation team to collaborate and address patient needs during the navigation process. Users benefit from a personal Dashboard alerting them to appointments, time-sensitive tasks, and incoming messages from patients. CONNECT’s report suite features over 50 reports that are filterable by 14 criteria selections, providing administrators with a comprehensive analysis of the navigation program. CONNECT can be integrated with Epic and Cerner Millennium utilizing FHIR technology facilitating access to the EHR and sending the Navigation Summary and Care Plan to the patient’s medical record. In addition to EHR interfaces, Nursenav and ERS (Electronic Registry Systems) have partnered and created an integration between our solutions to bring diagnosis and treatment data in from CRStar to CONNECT to expedite Survivorship Care Plan creation.

Care Plans
Care Plans in CONNECT are customized with your facility logo and colors. Survivorship Care Plans are delivered to patients with a treatable cancer and includes their diagnosis, how the cancer was treated, and instructions for follow-up care. Care Plans are provided to patients with a high-risk condition for cancer.

CONNECT calculates the next calendar date for each follow-up care instruction in a Care Plan or Survivorship Care Plan. The next date for the clinical care, screening and surveillance instruction is posted on the CONNECT user's s Follow-Up Care Alert calendar when it is due.

CONNECT’s report suite features over 50 reports that are filterable by 14 criteria selections, providing administrators with a comprehensive analysis of the navigation program. Advanced Reporting in CONNECT supports custom report creation for data analysis and time measures.
Online Secure Messaging and Electronic Distress Screenings
CONNECT provides navigation teams the convenience of communicating with patients online using CONNECT’s HIPAA-compliant Secure Messaging system. Secure Messaging is considered a type of encounter in CONNECT, allowing users to include referrals, education, and a digital version of their care plan when sending a message. Secure Messages are tracked as “read” or “unread” by the patient, confirming successful receipt by the patient.
Distress Screenings can also be sent to patients electronically, ensuring that assessments are presented to the patient at a ‘pivotal time’. This provides patients the convenience of completing the assessment in the comfort of their own environment. The navigator is notified on their Dashboard when the patient completes the assessment, and results are automatically documented in the patient record for the navigator to begin addressing any causes of distress reported by the patient.

CRStar Integration
For programs desiring to partner oncology navigation with the cancer registry, Nursenav and Electronic Registry Systems (ERS) have partnered and established an integration between our flagship solutions, CONNECT and CRStar. This integration benefits our mutual clients by receiving diagnosis and treatment data from CRStar into CONNECT after the abstraction is completed, expediting the time to create a Survivorship Care Plan. Clients also have the option of sending identified cancer cases in the suspense status to CONNECT, providing a patient referral stream for the navigation program.

FHIR Interface
Cerner Millennium and Epic clients have the option of interfacing CONNECT with their EHR utilizing FHIR technology available in these EHR platforms. Nursenav IT will work with the facility IT to establish connectivity between CONNECT and the EHR. The user’s level of access in the EHR, when accessed from within CONNECT, will honor the level established by the health system in the EHR. The FHIR Interface between CONNECT and the EHR will eliminate the need for dual logins and toggling between two software systems. Users will benefit from patient look-up in the EHR, one-click record creation, and access to diagnosis and treatment information. The FHIR interface also incorporates the navigation process into the patient’s medical record by sending the Navigation Summary and Care Plan from CONNECT to the patient’s medical record in the EHR.
